Transition Resource Sheet for Students with Disabilities

  • The purpose of this hyperlinked document is to support youth and families to make informed decisions about transition services for students with disabilities.
  • This transition resource sheet is to be shared with youth prior to and during 504/IEP meetings (ages 13/14 to 18/21).
  • The hyperlinked resources are targeted for use by transition age students and their families.  
  • The top three transition planning resources are listed in the first colored category on the document and then additional categories follow in alphabetical order.
  • The document can be printed as a one-page front and back resource.
  • At the end of the two-page document is a link “Please complete the Transition Resource Sheet Survey” Please encourage youth, families, and system partners to complete this link after using and or reviewing/sharing the document.
  • The document is being shared to test this resource sheet for at least three months beginning April 2025.
  • The goal is to take suggested edits and revise the document to be ready for the next school year (August/September of 2025)
  • At this point, the attached pdf resource has not reached the level of ADA accessibility that the workgroup would have liked. The goal is to have the revised document ready for the beginning of next school year to meet all accessibility requirements including by screen reader. We have created an accessible webpage version of this resource that is linked by hyperlink and QR code on the document.
